Provider Spotlight: Hazel Moore
The Kenosha Community Health Center (KCHC) has a multi-disciplinary team of healthcare providers collaborating on patients’ care to ensure that all served have access to high quality, compassionate and affordable care. Our Care Teams are made up of licensed board certified Medical Doctors and Nurse Practitioners, Registered Nurses, Licensed Practical Nurses, Licensed Clinical Social Workers and Licensed Practical Counselors.
One of the valued members of our dental team is Hazel Moore, Doctor of Dental Surgery.
Dr. Moore has worked in the Kenosha and Racine communities for 20 years. Dr. Moore began working at KCHC’s 22nd Avenue location in 1998. She became Dental Director in 2000 and moved to the Racine Community Health Center in 2008. Dr. Moore returned to her roots at KCHC in April 2015.
Dr. Moore began her dental career after graduating from McHarry Medical College, Nashville, TN, in 1977. “The experience that I bring is both academic and professional. Being a clinical provider we must constantly educate our patients as to the causes and prevention of dental disease, while we serve their dental needs,” she explained. While she has worked at a for-profit dental clinic, she finds working at community health centers personally fulfilling.
Dr. Moore enjoys working at KCHC and helping the underserved in Kenosha. “I believe that KCHC has a great impact on Kenosha County by helping to reduce the oral health problems and educate our patients on the causes and prevention of dental disease,” she stated.
Dr. Moore works at the KCHC branch located at 6226 14th Avenue, Kenosha, and is accepting new patients. Anyone interested in scheduling an appointment should contact the Kenosha Community Health Center at 262-236-7590.
KCHC provides medical, behavioral health and dental services and is a Federally Qualified Health Center. Learn more at www.kenoshachc.org.
